New State-Mandated Physical Forms To Be Used For 10-11 School Year

Starting with the 2010-2011 school year, all student-athletes will be required to utilize new physical forms provided by the IESA/IHSA. The new forms will include more information for your family physician to fill out. The new forms are part of a new legislative mandate from the State of Illinois.
For those students that will be getting an athletic physical prior to the end of school, please pick up a new form at the office or from Coach Stoecklin. New forms will be attached to year-end report cards for your use if a physical is going to be received over the summer.
